Start:

1. Create a new document with a black background, create a new layer 1, create a rounded rectangle, and use a silver gradient

2. Add drop shadow, inner shadow, inner glow and bevel relief layer styles to layer 1

3. Create a triangle selection and press DELETE to delete the internal color

4. Create a new layer 2 and use gradient inside the triangle selection

5. Select-Modify-Shrink [Shrink 4 pixels], then create a new layer 3 and use a white to transparent gradient

CTRL+D to cancel the selection and change the transparency of layer 3. The effect is as follows:

6. Use the Pen tool to create a path, and use the Convert Point tool to change the shape of the path, as follows:

Convert the path to a selection, delete the excess colors, and use CTRL+D to deselect
